The top commander in Afghanistan said Thursday he prefers a robust U.S. combat force of 68,000 in 2013, signaling a potential halt in the drawdown and complicating any effort by President Barack Obama to accelerate the timetable after more than a decade of war.
"My opinion is that we will need significant combat power in 2013," Marine Gen. John Allen told the Senate Armed Services Committee.
Pressed by the panel's top Republican, Sen. John McCain, on whether a force of 68,000 would be sufficient, Allen said, "Sixty-eight thousand is a good going-in number, but I owe the president some analysis on that."
In his second public hearing on Capitol Hill this week, the commanding general of the war in Afghanistan argued that the war can still be won, and he warned that a withdrawal of coalition forces would create a vacuum in the region.
"Should the U.S. leave Afghanistan, should ISAF, NATO leave Afghanistan, that would create in my mind, for all intents and purposes, a geopolitical vacuum," Allen said, as the Afghan National Security Forces prepare to take over responsibility.
Allen's comments represent a marker for the military. Obama faces increasing political and public pressure to accelerate the timetable after more than 10 years of fighting and recent incidents that dealt a major setback to the fragile U.S.-Afghanistan relationship. Afghan outrage over the burnings of Qurans and a shooting spree that left 17 Afghan civilians dead have been blamed on Americans.
Sen. Susan Collins, R-Maine, tried to get Allen to admit how much money the U.S. will have to pay to fund the Afghan National Security Forces over the next decade, but Allen said he could not say. He did acknowledge that the current annual cost to fund the ANSF's 352,000 troops is between $4 billion and $5 billion, but the entire annual revenue for Afghanistan is only $2 billion, so the international community will be helping the Afghans for some time.
Allen said he remains optimistic that the war can still be won.
"If I didn't think it was do-able I would tell you, and I would tell you very quickly because I wouldn't want to spend another life in this fight if it wasn't do-able," Allen said.
Asked later by Sen. Lindsey Graham whether the current strategy in Afghanistan represents "the last best chance for success in Afghanistan," Allen said quietly, "I concur with that, senator."

Tobacco use is responsible for almost half a million deaths in the U.S. each year, but the tobacco plant could find redemption as a savior for public health. That's because a U.S. biotechnology startup has transformed tobacco plants into living factories for making new vaccines and medical treatments.
The "SwiftVax" tobacco plants are designed to act as quick, cheap biological factories for churning out bioengineered proteins needed for human or animal vaccines. Faster vaccine manufacturing could allow the world to respond rapidly to future outbreaks of infectious diseases ? a problem it faced while racing to stockpile vaccine during the H1N1 swine flu pandemic.
"Thousands of lives and millions of dollars were lost because current technologies for production were not able to provide vaccines fast enough," said Lucas Arzola, a Ph.D. candidate in chemical engineering at the University of California, Davis. "The use of tobacco plants as biofactories of vaccines can provide a solution to this problem."
Arzola's startup, called Inserogen, has already begun lining up bioengineered products made by the SwiftVax tobacco plants. First, it's creating a vaccine for the poultry illness called Newcastle disease ? an infectious disease capable of rapidly spreading among chickens or other farm birds.
Second, a therapeutic protein called alpha-1 antitrypsin (AAT) could help human patients suffering from the genetic disorder known as AAT deficiency, because the missing protein typically protects the lungs and liver from damage. But Arzola envisions the SwiftVax technology making a wide variety of vaccines and proteins used in medical treatments.
"The technology is easy to customize, and can be engineered to produce not only vaccines, but virtually any recombinant protein," Arzola told InnovationNewsDaily. "A unique aspect of our technology is that we are repurposing tobacco, a plant that is viewed as negative, for a positive purpose: manufacturing vaccines and therapeutics to improve quality of life."
Arzola originally founded Inserogen alongside fellow Ph.D. candidates Oscar Ortega-Rivera and Michelle Lozada-Contreras, as well as Karen McDonald, a professor of chemical engineering at UC-Davis. Inserogen went on to win first place among 40 teams competing in the Big Bang! Business Plan Competition at UC Davis in 2010.
The startup's breakthroughs have built upon years of funding from the U.S. National Science Foundation ? it was accepted into the new Innovation Corps Program that awards a $50,000 grant to university teams. More recently, it received a $20,000 E-Team Grant from the National Collegiate Inventors and Innovators Alliance (NCIIA) to do a proof-of-concept for producing an animal vaccine.
Inserogen will also represent one of 14 student teams participating in the NCIIA's Open Minds exhibition being held at the Exploratorium museum in San Francisco on Friday, March 23. The startup team hopes to share tips with fellow entrepreneurs and perhaps find interested collaborators or investors.
Arzola had several tips for fellow entrepreneurs: build a team with complementary skills, don't be afraid of talking about the startup idea, and get involved with supporting organizations such as NCIIA.
"You must seek advice from experienced individuals who have built a similar company or have commercialized a similar technology or product before," Arzola said. "They have been there, done that, and you can learn a lot from their successes and mistakes. You will be surprised by their willingness to mentor and to provide valuable advice."

The languages your family uses: English and French
What motivated you to raise your child(ren) bilingually: I am from Switzerland and French is my first language; I want our children to learn/be familiar with both their parents? culture. My mother was bilingual French/Swiss-German but never taught us Swiss-German; I always regretted it.

How does the bilingualism work in your family (i.e., who speaks what language)? My husband speaks English and I speak French; though I do not always speak French; I do switch from French to English.
When do you switch? How do you decide on what language to use and when? I noticed that I do speak French to L. more easily and regularly when we are both home alone (I am a part-time stay at home mom). But I do always speak some French when my husband is around. I speak French when I feed him, change him, get him dressed, or bathe him, for instance. I noticed also that I often switch to English if there are people around; for some reasons I do not always feel comfortable speaking French when surrounded by English-speaking people (I don?t want to feel them excluded maybe?).
I do also discipline L. in English. My husband was raised with his mom saying ?not for baby? when there was something the kids were not supposed to do or go to? And I started using that expression too. I think that?s the reason why so far I have mostly been using English to discipline.
What are some of the challenges you?ve encountered? For some reasons, I don?t find it easy to speak to L. in French when in public (sometimes), but mostly around family. As I mentioned before, I think I?m worried that it may be considered rude by others.
Also, because I?m the only French speaker around here (my family is still in Switzerland and I don?t have anybody to speak French with), I am used to speaking in English all the time. So sometimes English is actually the easy to-go-to language. It?s what I?m used to, what my husband speaks, what I?ve been thinking in, writing in and even dreaming in for the past couple of year. I never thought I?d ever say this, but when I?m really tired at the end of a long day, English is often easier to use.
Finally, I do struggle between both languages. I?ve been living in the States for the past 6 years. I?m pretty fluent, but I still have an accent and sometimes I?m not sure of a word, or my pronunciation is not perfect. I am fluent in French, but because I don?t practice it as much anymore, sometimes I have trouble getting back into it. And writing is even more difficult. So there are days when I feel like I don?t have a first language anymore and I?m just struggling with both languages. And I don?t want this to affect our children. I don?t want them to learn both languages partially, with mistakes and all.

What have been some of the rewards of raising a bilingual child? L. does not speak yet, but he is starting to understand some things and responds to what is being said to him. It?s always rewarding when he reacts to what I tell him, whether it?s in French or English. Also, recently I?ve started to notice that I will automatically (and unconsciously) use French more often ? which was not the case when L. was first born. And I really enjoy this. It feels like I finally have someone I can talk to in French! The funny thing is, sometimes I do answer my husband in French or use French words in the middle of a sentence in English. I did not use to do this before L.
What role does your extended family play in your decision and ability to raise a bilingual child? My parents are very adamant that L. learns French. They live in Switzerland and I think they feel it?s a way for them to be close to him.
My mother-in-law is Hispanic. She did speak some Spanish to her kids, but never actually taught them the language. My husband and I talked about it and he mentions how he regretted that his mom did not really teach him Spanish. So it?s an encouragement for me.
What resources have you found useful? So far, I?ve gotten some children music in French and also lots of children books in French. That?s what I?ve mostly been using with L. One day, I also hope to get a kindle fire or something similar. I?m hoping it would make access to French literature and French movies for kids easier (and eventually cheaper too).
What are some of your long-term goals? I am going to homeschool our children, and I would like to homeschool them bilingual. I am not sure yet how I will handle things like math, science, or even history. But I know I want our homeschool to be both in English and French. I do think it will also be a great way to introduce our children to both our cultures (literature, history, arts, language, etc.)
What advice would you give a new parent starting the process? I am a new parent, so I don?t have any advice to give? but I?d love to get some from others!

By activating helicopter EMS from the field, or smaller hospital, EMS reduces time to treatment
CINCINNATIHealth care practitioners have long understood the importance of "door to balloon" time for heart attack patientsthe faster they can get the patient from the door of the hospital to a catheterization lab to open a clogged artery, the better the patient will do.
But a University of Cincinnati (UC) emergency medicine researcher says it's also important to study the "medical contact to balloon" time, acknowledging the role that emergency medical services (EMS) responders play in speeding patient care.
In a paper published online in the journal Academic Emergency Medicine, assistant professor Jason McMullan, MD, found that EMS responders can play a significant role in reducing medical contact to balloon (MCTB) time by activating helicopter emergency medical services (HEMS) earlier in the patient care process.
The research focused on patients with a ST-elevation myocardial infarction (STEMI), a type of heart attack caused by a sudden, total blockage of a coronary artery. The American Heart Association and American College of Cardiologists have set a goal of 90 minutes to open the artery by stent in a catheterization lab (known as percutaneous coronary intervention, or PCI).
"There's a saying doctors have when taking care of a heart attack patient: time is muscle," says McMullan. "This is because doctors race to open a clogged vessel in the heart to minimize permanent damage. The quicker you can open the clogged vessel, the more muscle you can save."
Small hospitals without PCI capabilities frequently use HEMS, such as UC Health Air Care & Mobile Care, to rush heart attack victims to larger hospitals for advanced treatment. But in a study published last year, McMullan and researchers found that hospital-activated HEMS transfer does not guarantee treatment within the 90-minute goal.
In this study, researchers from UC's Department of Emergency Medicine and UC Health Air Care found that if paramedics call for Air Care to meet them either at the smaller hospital or in the field, the time to get advanced care could be sped up by 48 minutes.
In multicenter retrospective chart review, the team studied cases of STEMI patients transferred by Air Care from local hospitals to PCI centers (hospital-initiated). They then modeled the potential medical contact to balloon times for two scenarios: if the EMS crews had requested HEMS transport directly from the scene to a PCI-capable hospital or if the EMS crew had the helicopter meet them at the local non-PCI hospital.
In the reviewed cases, the actual MCTB median time for 36 hospital-initiated transfers was 160 minutes, whereas the median MCTB time for the scene response scenario was estimated at 112 minutes. The median MCTB time for the hospital rendezvous scenario was estimated as 113 minutes.
"Both of the modeled strategies with EMS crews could result in meaningful time savings for patient care," says McMullan, "but neither strategy was superior, meaning individual communities can determine which method is best based on their unique needs and capabilities."
While the goal is to have every patient reach advanced care within 90 minutes, McMullan says researchers will continue to study multiple ways to reach the recommended time, including further study on the best transfer procedures and the benefits of giving STEMI patients fibrinolytics therapy at non-PCI hospitals before transfer.
"There is still a lot that smaller hospitals can do for heart attack victims," says McMullan, "and these hospitals will always be a strong link in the chain of care. But, if a person will need to be transferred for ultimate care, EMS activation of helicopter EMS can significantly speed up the process."

MONDAY, March 19 (HealthDay News) -- A new type of medication for type 2 diabetes helps to lower blood sugar levels when used in concert with insulin and other diabetes drugs, new research suggests.
The medicine is called dapagliflozin, and average blood sugar levels were lower in those taking the drug compared to those taking a placebo; both groups in the trial also took insulin and other diabetes medications. Daily insulin doses went down for those on the drug, and body weight dropped slightly.
"This study looked at the effects of dapagliflozin treatment in people with type 2 diabetes with high blood sugars despite insulin treatment, and found it was effective at reducing blood sugar, body weight and blood pressure," said study author Dr. John Wilding, head of the department of obesity and endocrinology at the University Hospital Aintree in Liverpool, England.
"Possible disadvantages include a slightly higher risk of urine infections and genital fungal infections, although most of these responded well to standard treatment," he added.
Results of the study are published in the March 20 issue of the Annals of Internal Medicine. The study was funded by AstraZeneca and Bristol-Meyers Squibb, two pharmaceutical companies who are collaborating in the development of dapagliflozin.
Type 2 diabetes affects the way your body metabolizes sugar. In type 2 diabetes, the body either doesn't use the hormone insulin effectively or it doesn't make enough insulin, according to the U.S. National Library of Medicine. Insulin allows the body's cells to convert sugar from food into fuel. If it isn't used well or there's not enough insulin, blood sugar levels will rise. High blood sugar levels can cause a number of serious health consequences, including damage to the eyes, kidneys and blood vessels.
In some cases, type 2 diabetes can be controlled with lifestyle changes, such as losing weight and exercising regularly. However, most people need medications to control their blood sugar levels, and many people need a combination of medications to lower their blood sugar.
Dapagliflozin is from a new class of type 2 diabetes medications that work by blocking the ability of the kidneys to hold sugar. This causes the kidneys to release sugar as waste in the urine.
Numerous clinical trials have found dapagliflozin to be effective at lowering blood sugar levels. Despite this beneficial effect, the U.S. Food and Drug Administration recently decided not to approve dapagliflozin until more information is available about its possible long-term side effects. The most significant concern cited by the FDA was a potential increase in the rates of bladder and breast cancer in people taking the drug.
Although the current trial wasn't long enough to look for longer-term outcomes, such as cancer, Wilding said that the slight increase in bladder and breast cancer was likely a chance finding. But, he said, because there is concern, it's important to continue surveillance for these cancers in future trials.
The current study was designed to see how the drug would work in combination with insulin and other type 2 diabetes medications.
The researchers enrolled just over 800 people with type 2 diabetes who were already taking insulin to control their blood sugar levels. In addition, the study volunteers could be taking up to two more blood sugar-lowering medications.
Study participants were randomly assigned to one of four treatment groups. They received either a placebo, or one of three doses of dapagliflozin (2.5, 5 or 10 milligrams) daily.
After 24 weeks of treatment, people in the dapagliflozin group lowered their average hemoglobin A1C level between 0.79 percent to 0.96 percent compared with a 0.39 percent reduction in those on placebo. Hemoglobin A1C is about a three-month average blood sugar level. People with diabetes are advised to keep their levels below 7 percent.
Insulin use also dropped for people taking the medication, suggesting more effective blood sugar control. And, body weight decreased between 2 pounds and 3.5 pounds for those taking the medication. Weight increased by almost 1 pound for those on placebo, according to the study.
In addition, the researchers found that both systolic and diastolic blood pressure levels went down more for those on the drug compared to those on placebo.
"In this study, dapagliflozin seemed to improve glucose control and lower the need for insulin, as well as cause some weight loss," said Dr. Sue Kirkman, senior vice president of medical affairs and community information of the American Diabetes Association.
"This is an interesting study on a new medication that's under development. It has a novel mechanism of action, but there have been some ongoing concerns about the safety of this class of medications, and none has been approved by the FDA yet," she noted.

ALGIERS, Algeria (AP) ? Algeria paid little formal attention to Monday's 50th anniversary of the cease-fire ending its bloody war of independence from France, and France officially ignored the awkward anniversary.
But some Algerians marked the date by renewing calls for the state to "criminalize colonialism" through the law.
Algeria's 132 years as a French colony have always dogged the relations between the two countries, which are close but continually marked by flare-ups over their shared past.
Monday marked the anniversary of the March 19, 1962, cease-fire, but neither country held big formal ceremonies.
Referring to French President Nicolas Sarkozy, the Algerian daily El Watan noted the lack of commemoration for the anniversary by both countries with the headline "Algeria is chilly, Sarkozy hides his face."
On Sunday the powerful National Organization of Mujahideen, those that fought in the 1954-1962 war of independence, pushed to re-energize a law that has languished in parliament for the past two years that would condemn France's colonial past.
The law's backers say it is a response to a 2005 law passed by the French parliament that some see as glorifying its time as a colonial power.
The National Liberation Front, the largest party in parliament and direct heir of the organization that battled the French, issued a statement putting forward its "immutable position" that France needs to apologize and acknowledge "its crimes against Algerians."
For its part, the French government announced Friday that there will be no official commemoration of the anniversary since it was "a date that divides and reopens deep wounds of a painful page in the recent history of France."
The statement, however, was not above some score-settling of its own.
"It also marks the beginning of the drama for those who were forcibly uprooted and the beginning of the tragedy for the harkis, massacred in the weeks that followed," said the statement. Algerians who fought on the French side were known as harkis.
